As an out-of-network provider, we can provide you with documentation – including any applicable service and diagnosis codes for you to submit to your health insurance for reimbursement. The provided documentation will also serve for payment utilizing Health Savings Accounts.

Contact us to schedule a virtual initial consultation with Dr. Jennifer Elfert. The fee for the initial hour-long consultation is $350. 

The Process:

1. Send in all testing, questionnaires, and forms that have been completed via the patient portal at least 2 days before your scheduled appointment.
2. During the appointment, Dr. Elfert will go over the results of the requested testing, questionnaires, and forms that you have provided. Dr. Elfert will then discuss the mechanism of your concerns and together will develop a treatment plan based on your specific symptoms and lifestyle.
3. Once a treatment plan is developed, an additional good faith estimate outlining associated fees will be provided for your approval.
4. Appointments with Dr. Elfert and one of our Certified Life Coaches will be scheduled. We have a proven protocol that consists of weekly and/or biweekly appointments with one or both of your practitioners.
